BENEFITS ALERT!
Subject: Benicorp Liquidation & Claim Settlements
Employees who had insurance coverage through Benicorp will receive a memorandum dated April 8, 2008, along with, a “Proof of Claim” Form. If you or your dependents have no outstanding claims, no action is required. However, if you have any outstanding claims, please complete the “Proof of Claim” and submit as instructed immediately.
The Liquidation Court has set May 15, 2009 as the “Claims Bar Date”. Claims postmarked after this date, will not be considered for payment.
If you have any questions regarding this matter or need assistance in filling out the “Proof of Claim” form, please contact your Benefits Department at 317-549-2000. We will be happy to help you.
